Nikki Parquette, MPAS, PA-C

After Nikki Parquette graduated from Illinois College with Bachelor of Science in Exercise Physiology, she saw U.S. Soldiers were dying on the battlefield because there were not enough combat medics providing care to Soldiers at the time of injury. Knowing she could increase that access, she enlisted in the U.S. Army as a combat medic and provided much needed support to our troops.
Once she was in the medical system, she realized that too many sick and hurting people were receiving inferior and impersonal care. So, in 2014, she earned a Master’s Degree through the Interservice Physician Assistant Program with the University of Nebraska Medical Center in Physician Assistant Studies and years later, opened Spear Direct Care. The purpose of Spear is to provide easy access to superior primary care when needed at a flat known price.
Spending years working in the primary care realm, Nikki realized that the mental health, and more importantly the brain health, epidemic was partly due to traditional medicine’s inability to help the brain heal itself. So North Idaho TMS & Brain Center was developed for the sole purpose of providing multiple modalities to enable people to help their brains heal leading to improved mental health.
Nikki and her husband Tom moved to Coeur d’ Alene in 2015, where they had their son Garrett. She enjoys spending time with her family (including their 2 dogs Jax and Sadie), weight lifting, dancing, hip-hop music, long drives, and delicious food.

Gayle Smith, D.O.

Originally from the East Coast, Dr. Gayle Smith moved to the beautiful inland northwest in 2009 after completing her residency in Maine, working for a brief stint in Alaska above the arctic circle, and practicing with an amazing crew in an FQHC on Chicago’s south side. She worked for many years at Dirne Community Health center (later Heritage Health), learning much from her patients, her peers, and a tireless team of nurses and medical assistants.
While always concerned with the challenges of depression and anxiety disorders, it was in this family practice setting that Dr. Smith became so acutely aware of the dire lack of effective resources for so many who struggle with varying degrees of mental illness. Initially, lifestyle medicine and disease prevention were the main focus of Dr. Smith’s practice, but it was discouraging to see countless patients whose depression did not improve despite changes in nutrition, exercise, and attention to spirit.
Dr. Gayle first became aware of TMS therapy through a patient’s inquiry – at that time TMS was still a treatment used in university settings, but Dr. Gayle continued to follow its integration into outpatient clinical practice over the last 10 years. It was exciting for her to see a technology that so effectively improved neurologic functioning for a condition that seemed so resistant to standard treatments! As you can imagine, Dr. Gayle is delighted to play a role at North Idaho TMS & Brain Center talking with patients and their families about this remarkable treatment that supports better brain function for all patients.


Nicole Chaney
Certified Health Coach
Nicole is a Certified Health and Wellness coach. She is excited to provide education about TMS therapy as well as how people can make powerful behavioral shifts that lead to greater life satisfaction, less dis-ease, and better emotional and physical performance. Nicole grew up in Post Falls, ID and enjoys her garden, hiking, and playing with pup Nala.
